2. Present Request Form/Order of Payment and other documents at the designated Window.
3. Present Order of Payment to the Cashier and pay for certification / authentication fees;
4. Present all documents and Official Receipt to the designated window.
5. Sign in the releasing logbook/ log sheet and receive certification /authenticated documents.

For: DFA AUTHENTICATION:
- Same procedures shall be followed. At the end of transaction, Client receives the original PRC documents and DFA
- Claim Slip issued by Records/Registration Officer. The authenticated photocopies of the original PRC documents shall be forwarded by the Records Officer to the DFA Office. Client shall claim the authenticated copies from the DFA office in Manila on the date specified in the Claim Slip.

The Professional Regulation Commission announces that Nurses Licensure Examination in San Fernando City, La Union on November 29 and 30, 2009 will be administered only to examinees from La Union, Ilocos Norte and Ilocos Sur. Examinees from Pangasinan will take the examination in Baguio City.

The NLE in La Union has been approved by the Commission so that these examinees will not experience any risks, inconveniences and additional travel costs in going to their testing centers in case of calamities.

Room assignments have been posted at http://prcbaguio.blogspot.com and will be posted at testing centers on November 28, 2009. Room assignments are final and are based on permanent addresses indicated in the Application Form.
